Funded under Horizon Europe’s Innovative Health Initiative (HORIZON-JU-IHI-2022-02-01) and running from 2023 to 2028, the iCARE4CVD project aims to improve patient care across the board, starting from those at risk of developing cardiovascular disease to those with advanced heart failure. Thomas More University of Applied Sciences plays an important role in the project focusing data analysis and AI modeling as well as on the implementation of Patient-Reported Outcome Measures and Patient-Reported Experience Measures. Flanders Research Institute for Agriculture, Fisheries and Food (EV ILVO) is partner in the project consortium and leads the work package on the development and dissemination of practical tools to help stakeholders prepare successful multi-actor proposals.
